WIN is a program to help people who have little or no health insurance get the care they need. WIN is improving health care access for the uninsured and underinsured in the City of New Haven.What does WIN do? WIN helps uninsured and underinsured people get the health care they need in several ways:
How does WIN work?
- Case managment - WIN has case managers at each agency to help individual patients coordinate their health care needs. This includes finding a primary care provider for each patient, helping patients schedule appointments for follow-up visits and helping them apply for healthcare coverage.
- Information network - A computer network links the participating agencies, allowing them to share essential patient information, while still protecting the confidentiality and privacy of the patients.
- Sliding fees WIN sets sliding fees to so each patient can afford the services.
- Patient education- WIN case managers educate patients about illness and injury prevention and the importance of getting medical care in a timely manner.
- Patients who go to the emergency departments at Yale-New Haven Hospital and the Hospital of Saint Rapahel will be treated and asked if they want to participate in WIN
- Patients who agree to participate in WIN will get an appointment to see a case manager who will guide them through the next steps in following up on their current medical situation, as well as help them with preventice care.
