Brief History of The Greater New Haven
Partnership for a Healthy Community
The Partnership originated in 1995 as a coalition of
community-based organizations, civic, health and
business organizations, private citizens, and
municipalities working together to enhance the health
and well being of residents of Greater New Haven.
Institutional members include:
The Community Foundation for Greater New Haven, Yale-New
Haven Hospital, Visiting Nurse Association of South Central Connecticut, The New
Haven Chamber of Commerce, Community Action Agency, Fair
Haven Community Health Center, the Hill Health Center,
the Yale University School of Epidemiology and Public Health and the City
of New Haven Health Department. The partnership achieved the 501C3 status in 2004.
Brief Statement of Organization's Mission:
The mission of the Partnership is
to ensure collaborative and accountable community
efforts to remove barriers and build a healthy, safe and
self-sufficient community in Greater New Haven.
The Goals of the Partnership are:
Improve the health status of the Greater New Haven Community
Gain a comprehensive understanding the community's health status and potential
Marshall resources, link existing resources, and if neccessary,
develop new resources to improve the quality of life in Greater New Haven
Empower people to take greater responsibility of their own health status
Develop a community health improvement action plan
Create a unified voice in order to influence policy and secure
funding for health related initiatives
Description of Current Programs
The Partnership is involved in the development of two primary projects:
1) ACCESS: Reducing Health Disparities: 100% Access, 0% Disparities, Specialty Care and
2) Healthy Homes: An Asthma Prevention, Intervention Project and presently exploring Universal Health Care in Connecticut.
